Live Scan fingerprinting

Every parent who volunteers with students — room parents included — clears a Live Scan fingerprint background check first. It's one appointment, usually about fifteen minutes. This page walks the whole thing start to finish so you only have to make the trip once.

The form is the California DOJ's Request for Live Scan Service (BCIA 8016). Fairmont has already filled in the school's half — you only complete the applicant section.

The short version

1. Print the form and fill in only the "Applicant Information" box.
2. Take it to a Live Scan site with unexpired photo ID and about $20 for the rolling fee.
3. Email us the stamped copy they hand back. That's it — results go to the school directly.

Before you go — three things in hand

1. The printed form, applicant section filled in

Download it here and print it single-sided. Fill in the Applicant Information box only: your name, date of birth, sex, height, weight, eye and hair color, place of birth, driver's license number, Social Security number, and home address.

Leave everything else exactly as printed — the Contributing Agency block, the Billing Number, the Level of Service checkboxes, and the shaded "Live Scan Transaction Completed By" strip at the bottom. That last strip is the operator's to fill in, not yours. If a field in the top section is blank, leave it blank rather than guessing; the ORI code already on the form tells the operator everything they need.

2. Valid, unexpired photo ID

Driver's license, state ID, or passport. Expired ID will not be accepted — it's the most common reason people get turned away and have to come back.

3. Money for the rolling fee — and only the rolling fee

This is the part that trips most people up, so it's worth being precise. Two different charges exist:

• Government fees — the state (DOJ) and federal (FBI) record checks. Fairmont's billing number is already printed on your form, which means these are billed straight to the school. You don't pay them.
• The rolling fee — what the operator charges to physically take the prints. This one is yours, and it varies by location. Fairmont's preferred site charges $20.00.

You'll see some sites on the state's list marked "BNR" — Billing Number Required. Those sites only serve applicants whose form already carries an agency billing number, and they collect the rolling fee and nothing else. Your form has a billing number, so BNR sites are open to you and are usually the cheapest option. Don't skip past them.

Where to get it done

Closer to campus

Fairmont's preferred site is up in Anaheim, which is a haul from San Juan Capistrano. There are certified operators much closer, and any of them can do the job — the form works anywhere on the state's list. Start with the DOJ's Orange County list:

Then call before you drive out. Hours shift, some sites are appointment-only, some have moved, and rolling fees range from roughly $20 to $40. Two minutes on the phone saves a wasted trip. Worth asking: are you taking walk-ins today, what's your rolling fee, and are you set up for a form that already has an agency billing number on it?

At the appointment

Hand over the form and your ID. The operator scans your fingerprints electronically — no ink, no card to mail. The whole thing runs about ten to fifteen minutes.

They keep the original, complete the bottom strip, and hand you back a copy stamped with an ATI number. That number is your proof the submission went through. Don't lose it — it's the only receipt in the process, and if anything needs chasing down later, it's the first thing anyone will ask you for.

Afterward

1. Email us a photo or scan of the stamped copy — the link opens a message with the details we need already laid out.
2. Results go from the DOJ straight to Fairmont. You won't get a copy, and neither does the FPA — we only hear whether you're cleared.
3. Clearance usually lands within a few days, though it can take longer. Get fingerprinted well ahead of the event you're volunteering for rather than the week of.

Questions people actually ask

I was fingerprinted for my job. Doesn't that count?

No. Live Scan results don't transfer between employers or institutions — each agency's request is tied to its own ORI code. Prints you had done for another company, another school, or a volunteer role elsewhere have to be redone for Fairmont Private Schools, on Fairmont's form.

Do I need an appointment?

Depends on the site. Fairmont's preferred location takes walk-ins six days a week. Others are appointment-only. Call ahead.

What if the site asks me to pay the DOJ and FBI fees?

Point them to the billing number printed in the Applicant Information block of your form — those fees bill to Fairmont. If a location still insists on charging you, don't pay it; call the campus office at (714) 234-2700 or try a different site.

My form got rejected and I need to redo it.

A resubmission needs the original ATI number written in the "If re-submission" box, plus proof of the rejection. Email sjc-treasurer@fairmontpa.org and we'll help you sort it out before you go back.
